Client Information Form (Tattoo)

* I am over the age of 18 years.
* I have been given full instruction regarding caring for my tattoo, and fully understand my responsibilities for aftercare.
* I have been given the opportunity to ask any questions and I am satisfied that all my questions have been answered.
* I understand the possibility of reaction or infection due to the inks used.
* I release PEPPERMINT STAR PTY LTD ATF STAR TRUST and any of it's Directors or Employee's of all or any liability that may arise out of any damages,
whether physical or psychological, arising directly or indirectly out of a result to my tattoo (Without limiting the causes for my damages which may arise out of obtaining my tattoo.
Theses may include: Design, shape and size, location of my tattoo, infection arising from my tattoo and/or violence, injury or other damages arising from the existence of my tattoo.).
* I have checked and approve the design and spelling of the stencil.
* I agree to the placement of the tattoo.
* I understand that the artist is not responsible for doing finger, inner lip, and/or hand/feet tattoo touch-ups free of charge, due to the fact that the client is aware that the tattoo will be on a surface area that fades quickly.
